Top 5 Nerve Supplements Ingredients Researchers Study
The top 5 nerve supplements ingredients that researchers study are vitamin B12, benfotiamine, alpha lipoic acid, palmitoylethanolamide (PEA) and acetyl-L-carnitine. The list shows what gets studied most, not what works best. Evidence is mixed, trials are small, and most work was done in diabetic neuropathy, not sciatica.

What are the top 5 nerve supplements ingredients?
The top five ingredients are the ones that appear most often in nerve supplement trials and on nerve supplement labels. They are B12, benfotiamine, alpha lipoic acid, PEA and acetyl-L-carnitine.
This is a list by research attention, not a ranking of results. No ingredient below has strong proof for sciatica.
| Ingredient | Trial dose | Evidence in short |
|---|---|---|
| Vitamin B12 | Fix a deficiency | Deficiency can cause numbness and tingling |
| Benfotiamine | 300 to 600 mg a day | Small diabetic neuropathy trials |
| Alpha lipoic acid | 600 to 1,800 mg a day | Symptom scores up, pain scores not |
| PEA | 300 to 600 mg a day | Small trials, some industry links |
| Acetyl-L-carnitine | 1,500 to 3,000 mg a day | Low-certainty evidence |
Why are B12 and benfotiamine on the list?
B12 and benfotiamine are on the list because both relate to nerve function. Low B12 is a known cause of numbness and tingling in the hands and feet, sometimes without anemia (NIH ODS, Vitamin B12).
That is a deficiency story. If your B12 is normal, extra B12 has no proven nerve benefit. Metformin and acid-reducing drugs can lower B12, so ask your doctor for a blood test.
Benfotiamine is a fat-soluble form of thiamin. Three small placebo-controlled studies in diabetic neuropathy found milder symptoms, but the NIH says larger studies are needed (NIH ODS, Thiamin). One six-week trial saw gains in one symptom score, not all of them (Stracke et al., BENDIP 2008).
One safety note on B vitamins as a group: do not stack products that each contain B6. Very high B6 intake can itself harm nerves (NIH ODS, Vitamin B6).
What do researchers say about alpha lipoic acid, PEA and carnitine?
Researchers describe all three as promising but unproven for nerve pain. The studies are small, short and often tied to makers.
Alpha lipoic acid
A review of 10 trials found better symptom scores at 600 to 1,800 mg a day. Pain and nerve tests did not improve (Hsieh et al., Nutrients 2023).
PEA
A review found about a 2-point drop on a 10-point pain scale. The authors flagged poor study quality (Artukoglu et al., Pain Physician 2017). A 3-week sciatica trial used 300 or 600 mg a day (Cruccu et al., CNS Drug Targets 2019).
Acetyl-L-carnitine
A Cochrane review was very uncertain. At 1,500 mg or less there was no difference from placebo (Rolim et al., Cochrane 2019).
Read the doses before you trust a label. A trial dose and a capsule dose are often far apart.

See packsHow do you judge a nerve supplement that uses these ingredients?
Judge it by the printed amounts, the serving size and the quality of the sources, not by the ingredient names.
- Every line should show an amount, not only a blend total.
- Compare each amount with the trial dose in the table above.
- Skip any label that promises to fix your nerves.
- Ask your doctor or pharmacist if you take blood thinners, gabapentin, diabetes drugs or sedatives.
Our nerve supplements guide and ingredients page go deeper, and the B vitamins page covers the vitamin lines.
As one example, SciatiEase holds 1 mg of methylcobalamin B12, 200 mg of benfotiamine, 250 mg of R-ALA, 500 mg of PEA and 250 mg of acetyl-L-carnitine per 4 capsules. PEA sits in the studied range; the others sit below trial doses. It was not tested as a product.

FAQ01
Quick answers on nerve supplement ingredients
Which nerve supplement ingredient has the most evidence?
Vitamin B12 has the clearest evidence, but only for correcting a deficiency, which can cause numbness and tingling (NIH ODS, Vitamin B12). For the other four, trials are small and mixed, so none has strong proof for sciatica.
Do you need all five ingredients in one capsule?
No. A formula with five ingredients is not stronger than one with two. What matters is whether each amount is printed and close to a studied dose. Ask your doctor what suits your situation.
Are these ingredients proven for sciatica?
No. Most trials were in diabetic neuropathy, and the one PEA sciatica trial lasted 3 weeks (Cruccu et al., CNS Drug Targets 2019). Sciatica often improves in weeks regardless of care (NHS, Sciatica), so results are hard to credit to a supplement.
