Sciatica vs Neuropathy: Two Nerve Problems Compared
Sciatica vs neuropathy comes down to scope. Sciatica is pain or numbness from one irritated sciatic nerve, usually down one leg. Peripheral neuropathy is damage to nerves outside the brain and spine, often felt in both feet first. Diabetes is its leading cause. A doctor can tell them apart.

What is the difference between sciatica and neuropathy?
Sciatica involves one nerve, the sciatic nerve, irritated near the spine. Peripheral neuropathy is damage to nerves outside the brain and spinal cord, and it often affects many nerves (NINDS, Peripheral Neuropathy).
Sciatica is a symptom, with common causes such as a herniated disk or spinal stenosis. Neuropathy has many causes, and diabetes is the leading one.
NINDS lists sciatica as an example of neuropathic pain, which blurs the words a little (NINDS, Pain). That is why the pattern matters more than the label.
| Point | Sciatica | Peripheral neuropathy |
|---|---|---|
| Where | Back, buttock, one leg | Often both feet, then legs and hands |
| Typical feel | Sharp, shooting, tingling | Burning, tingling, numbness |
| Common causes | Disk, narrowing, injury | Diabetes, low B12, other conditions |
| Often worse | Sitting, coughing | At night |
What causes each one?
Sciatica is usually mechanical, meaning something presses on the nerve. Neuropathy is usually a body-wide issue that harms nerves over time.
About two-thirds of people with diabetes have mild to severe nerve problems. NINDS also names vitamin B12 deficiency and too much vitamin B6 as the best-known vitamin causes.
B12 deficiency can cause numbness and tingling in the hands and feet (NIH ODS, Vitamin B12). Metformin and acid-reducing drugs can lower B12 absorption, so ask your doctor about a blood test if you take them.
- Sciatica: disk herniation, spinal stenosis, injury
- Neuropathy: diabetes, B12 deficiency, excess B6, other illnesses
- Either: sometimes no clear cause is found
How do doctors tell sciatica from neuropathy?
Doctors start with where the symptoms are and what you feel. One-sided pain from the back down the leg points toward sciatica. Burning or numb feet on both sides points toward neuropathy.
They may add blood tests for blood sugar or B12, a nerve exam or imaging. You can have both conditions at once.
Write which body parts are affected, when it is worse and every medicine and supplement you take. It speeds the visit.
For diabetes, daily foot checks and a yearly foot exam are advised. Pain medicines may ease symptoms but do not repair nerves, and NIDDK advises talking with a doctor before using supplements.

The evidence is mixed and mostly limited to neuropathy, so no supplement should replace a diagnosis. Oral alpha-lipoic acid trials gave mixed results (Linus Pauling Inst., Lipoic Acid).
SciatiEase is one example. Per 4 capsules it lists R-ALA 250 mg, PEA 500 mg, ALCAR 250 mg, benfotiamine 200 mg and B vitamins. No trial has tested SciatiEase itself, and several of its doses sit below study doses. Read the nerve supplements guide and the sciatica basics guide first.
Both legs weak, numbness around the groin or bottom, bladder or bowel changes, or symptoms after an injury need emergency care.
Keep taking prescribed medicine, and ask your doctor or pharmacist before adding anything.

FAQ01
Quick answers on sciatica and neuropathy
Can neuropathy cause leg pain like sciatica?
Yes. Neuropathy can cause burning, tingling or pain in the legs, and the feel can resemble sciatica. The usual difference is that neuropathy often hits both feet first. A doctor can tell which fits.
Is sciatica a type of neuropathy?
Not exactly. Sciatica is a symptom from an irritated nerve root or nerve, while peripheral neuropathy describes damage to nerves outside the brain and spine. NINDS groups sciatica under neuropathic pain, so the terms overlap.
Which one is more common?
Back-related pain is common, but exact sciatica numbers vary widely by study. For neuropathy, diabetes is the leading cause, and about two-thirds of people with diabetes have nerve problems of some degree.
