Sciatica vs Back Pain: How to Tell the Difference

Quick answer

Sciatica vs back pain comes down to where you feel it. Back pain stays mostly in the lower back. Sciatica sends pain, tingling or numbness along the sciatic nerve, often from the back down one leg. Only an exam can confirm which you have, and both can occur together.

What is the difference between sciatica and back pain?

Back pain is pain in the muscles, joints or disks of the back. Sciatica is pain that follows the sciatic nerve, often from the lower back down one leg.

Back pain is very common. In 2019, 39.0% of U.S. adults reported back pain in the past three months (CDC NCHS Data Brief 415). Sciatica is less common, and studies disagree on how many people have it (Konstantinou & Dunn, Spine 2008).

Can you have sciatica and back pain at the same time?

Yes. Many people have both, because a problem in the lower spine can irritate muscles and the nerve together.

One review describes sciatica as more persistent and more severe than low back pain alone, with a less favorable outcome. That is why leg symptoms are worth noting early.

The cause can also be the same. A herniated disk, spinal stenosis or an injury can lead to either one. Scans can confuse the picture. Disk bulges appear in 30% of pain-free 20-year-olds and 84% of 80-year-olds (Brinjikji et al., AJNR 2015).

  • Back pain with no leg symptoms is likely a back issue
  • Leg pain below the knee points more toward the nerve
  • Both together are common and worth a doctor's look

Is the care for sciatica different from back pain care?

Early care is often similar. For most low back pain, the American College of Physicians suggests starting with heat, massage, acupuncture, spinal manipulation or exercise.

For sciatica, the NHS adds staying as active as possible and doing regular sciatica exercises. Medicines are less clear. A review of sciatica drugs found low to moderate quality evidence and little advantage over placebo for most (Pinto et al., BMJ 2012). Gabapentinoids showed no benefit for back or radicular pain in nine trials.

Match care to the cause

The right plan depends on the diagnosis. Your doctor decides which exercises, medicines or tests fit you.

When should either one get medical attention?

Either one needs attention if pain does not improve after a few weeks, keeps getting worse or stops normal activities.

Emergency care, not a wait

Pain in both legs, leg weakness, numbness around the groin or bottom, bladder or bowel changes, or pain after an injury.

Keep taking prescribed medicine. Ask your doctor or pharmacist before you add a supplement, mainly if you take a blood thinner, gabapentin, diabetes drugs or sedatives.

SciatiEase is a capsule supplement that is not tested for either problem, and it does not replace care. Read who SciatiEase is for and the sciatica basics guide before deciding.

FAQ01

Quick answers on sciatica and back pain

Does sciatica always hurt below the knee?

No. Pain can stop at the buttock or thigh, or travel to the foot. Reaching below the knee is a common sign of nerve involvement, but the path varies, so tell your doctor exactly where you feel it.

Can back pain turn into sciatica?

It can. A disk problem or narrowed spinal canal that first causes back pain may later press on a nerve root and send symptoms down the leg. New leg pain or tingling is worth mentioning to a clinician.

Is sitting worse for sciatica or back pain?

It can be worse for either. Many people with sciatica say long sitting brings on leg pain, while back pain may ease when moving. Short breaks to stand and walk are a simple thing to try.
