Sciatica basics

Sciatica Causes: Discs, Stenosis and Other Triggers

Quick answer

Sciatica causes include a herniated disk, spinal stenosis and injury, though often no cause is found. Sciatica is a symptom of a problem with the sciatic nerve, not a disease. Pain, numbness or tingling often runs from the lower back down one leg. Both legs, weakness or bladder changes need urgent care.

What are the most common sciatica causes?

The most common sciatica causes are a herniated disk, spinal stenosis and injury (MedlinePlus, Sciatica). Sciatica is a symptom of a problem with the sciatic nerve, not a disease itself.

Many cases have no clear cause. That can be frustrating, but it does not mean the pain is not real.

Common triggers of sciatica
CauseWhat happens
Herniated diskDisk material presses on a nerve root
Spinal stenosisThe spinal canal narrows and squeezes nerves
InjuryTrauma to the back, hip or pelvis
No cause foundCommon; symptoms may still pass

Does a disc bulge on a scan explain sciatica?

Not always. A disk bulge on a scan does not prove it is the source of your pain.

A review of 33 studies found disk bulges in 30% of 20-year-olds without back pain, rising to 84% of 80-year-olds (Brinjikji et al., AJNR 2015). The authors say many findings are part of normal aging and must be read alongside symptoms.

That is why a doctor looks at your history and exam first. A scan helps most when it fits what you feel.

Back pain is also common. In 2019, 39.0% of U.S. adults reported back pain in the past 3 months (CDC NCHS Data Brief 415). Reported sciatica rates vary widely, from 1.2% to 43%, because studies define it differently (Konstantinou & Dunn, Spine 2008).

How long does sciatica from these causes last?

Sciatica usually gets better within a few weeks to a few months, though it can last longer and can return (NHS, Sciatica). Some cases go away on their own (MedlinePlus, Sciatica).

The NHS advises staying as active as possible, doing sciatica exercises and using heat packs (NHS, Sciatica). The American College of Physicians also suggests starting with non-drug care for back pain, such as heat, massage or exercise (Qaseem et al., ACP guideline 2017).

  • Keep moving within comfort.
  • Use heat for sore muscles.
  • See a GP if pain has not improved after a few weeks or keeps getting worse.

Keep taking prescribed medicine. A supplement does not change the cause.

Which sciatica symptoms need urgent care?

Some symptoms need emergency care at once. They can signal cauda equina syndrome, where nerve roots at the base of the spine are squeezed (AANS, Cauda Equina Syndrome).

Go to emergency care for

Sciatica in both legs, numbness around the groin or bottom, new bladder or bowel changes, weak legs, or pain after an injury.

Urgent surgery is usually the treatment for cauda equina syndrome, and acting within 48 hours helps recovery (AANS, Cauda Equina Syndrome). Do not wait for it to pass.

FAQ01

Quick answers on what causes sciatica

Can sitting too long cause sciatica?

We did not find a source in our library that links sitting to sciatica, so we make no claim. Staying as active as possible is the NHS advice for recovery (NHS, Sciatica). Ask your GP if sitting seems to worsen your pain.

Is sciatica the same as low back pain?

No. Low back pain is pain in the lower back. Sciatica is pain, numbness or tingling that often runs down one leg from a nerve problem. Reviews describe it as more persistent and severe than back pain alone (Konstantinou & Dunn, Spine 2008).

Can sciatica go away without treatment?

Yes, sometimes. MedlinePlus says it sometimes goes away on its own (MedlinePlus, Sciatica), and the NHS says it usually improves in weeks to months (NHS, Sciatica). See a GP if it has not improved after a few weeks.
