Alpha Lipoic Acid Sciatica Research: What Exists
Alpha lipoic acid sciatica research is thin. We found no human trial of oral ALA for sciatica in our sources. The best-studied use is diabetic neuropathy, at 600 to 1,800 mg a day. Those results do not transfer to sciatica, and SciatiEase lists 250 mg of R-ALA per serving.

Is there research on alpha lipoic acid for sciatica?
We found no human trial of oral alpha lipoic acid (ALA) for sciatica in the sources we checked. The research that exists studies other nerve problems.
Most human work looks at diabetic neuropathy, which is nerve damage from high blood sugar in the feet and legs (NIDDK, Peripheral Neuropathy). Sciatica is different. It usually comes from pressure on a nerve root in the spine (MedlinePlus, Sciatica).
Some animal and lab studies look at ALA and nerves too. They show what to test next. They do not show what happens in people.
| Condition | Type of evidence | Dose |
|---|---|---|
| Diabetic neuropathy | Human trials, IV and oral | 300 to 1,800 mg a day |
| Sciatica | No human trial found | Not applicable |
What do diabetic neuropathy studies say about ALA?
Diabetic neuropathy studies say that ALA helps symptom scores more than pain scores. A pooled review of 10 trials and 1,242 people tested oral ALA at 600, 1,200 and 1,800 mg a day (Hsieh et al., Nutrients 2023).
Total symptom scores improved. Pain scores, vibration sense and nerve conduction did not. Reviews found IV doses of 300 to 600 mg clearly reduced symptoms, while oral results were mixed. Many trials came from one group funded by an ALA maker (Linus Pauling Inst., Lipoic Acid).
So even where ALA is best studied, the result is mixed.
Diabetic nerve damage and a pinched spinal nerve are different problems. A result in one does not carry to the other.
How does SciatiEase's alpha lipoic acid amount compare?
SciatiEase lists 250 mg of R-alpha lipoic acid per serving of 4 capsules. At 2 capsules, that is 125 mg.
Both are well below the 600 mg or more used in the trials. Supplements usually hold 50 to 600 mg (Linus Pauling Inst., Lipoic Acid), so 250 mg is within the common range. It is not a studied dose for nerves.
R-ALA is one form of the molecule. We did not verify which form each trial used, so a direct comparison is not possible. The brand's own page lists 300 mg, but the panel says 250 mg, and we follow the panel.
- 4 capsules: 250 mg R-ALA.
- 2 capsules: 125 mg R-ALA.
- Trial doses: 600 to 1,800 mg by mouth.

See packsWhat should you ask your doctor about ALA and sciatica?
Ask your doctor whether your sciatica has a known cause and what first-line care fits it. Most sciatica improves in a few weeks to a few months (NHS, Sciatica). Exercise, heat and staying active come first.
ALA can lower blood sugar, so people on insulin or diabetes pills should ask before using it (Linus Pauling Inst., Lipoic Acid). Keep taking prescribed medicine, and do not swap it for a supplement.
Go to emergency care for pain in both legs, numbness around the groin or bottom, bladder or bowel changes, weak legs, or symptoms after an injury.

FAQ01
Quick answers on ALA and sciatica
Does alpha lipoic acid help a pinched nerve?
We found no human trial showing that it does. Oral ALA has been tested in diabetic neuropathy, not in pinched spinal nerves. Studies in one condition cannot be assumed to apply to the other (Hsieh et al., Nutrients 2023).
What is the difference between R-ALA and plain ALA?
R-ALA is one mirror-image form of alpha lipoic acid. Plain ALA is usually an equal mix of two forms. Our sources do not compare them in sciatica, so we cannot say one works better.
Why do IV and oral ALA trials differ?
Reviews found IV doses of 300 to 600 mg clearly reduced symptoms, while oral results were mixed (Linus Pauling Inst., Lipoic Acid). The two routes are not interchangeable. SciatiEase is an oral capsule.
